This includes self-written documents of all service-related information such as; Units attached to, Awards/Citations, Deployments/ Conflicts, and dates that can be verified. If you are a family member submitting these documents please note that a DD 214 is a military Certificate of Release or Discharge from Active Duty form. This document contains a service member's history. A DD 256 certificate of honorable service will also be accepted.
